Please type in the donor ID number(s) in the "Ordering Instructions" if you wish to purchase more than one, or if the donor number you want is not available in the drop down selection. Price includes shipping by two day air letter within the USA. All international requests will be sent via regular mail service. You must be currently registered with NW Andrology & Cryobank in order to purchase these extended donor histories. (see: http://www.nwcryobank.com/release_forms.asp) There is no charge associated with registration. By purchasing a donor history, clients agree not to redistribute donor history information to any third party as the accuracy of the information would then be questionable and not directly provided through the Cryobank. If you are purchasing histories in anticipation of purchasing specimens, please call and check inventory prior to purchase. It is possible to purchase a donor history and then find that the donor has sold out. As donor inventory is very dynamic, we are unable to offer refunds on histories because of donor inventory shortages. |

Monthly Storage of Sperm Specimens $35.00 Monthly sperm specimen storage. This product is only for paying monthly storage fees on line by credit card. This product should only be purchased if you: -are already a registered client -already have specimens in short term storage with our facility, and -you already have a short term storage agreement on file with us. Do not pay for more storage than you need. These storage charges are non refundable. Should you need storage for many months, then you should consider long term storage to help save money and to avoid monthly billing. |
